Flight itinerary
Include a copy if you have a confirmed booking. Not required if your travel dates are still being finalized.
Three visa photos
Plain white background, full face forward, taken within the last month. Myanmar requires three photos — one more than most countries. They are taken at the Sameday Passport & Visa office.
Traveling with a minor under 18?
Bring the child's birth certificate, copies of both parents' passports or driver's licenses, and a notarized parental consent letter if one parent is not traveling.

What the business visa needs
- Work history form. Provided at the Sameday Passport & Visa office. It summarizes your professional background for the consulate. No separate employer letter or business invitation from Myanmar is required.
- Three visa photos. Plain white background, taken within the last month. Myanmar requires three photos. They are taken at the office along with the Report Arrival Form.
- Flight itinerary. Include a copy if you have a confirmed booking. Not required if travel dates are still being finalized.
- Applying by mail? Include a completed Service Order Form so Sameday Passport & Visa can contact you quickly if anything needs clarifying before submission.
Where Myanmar applications go wrong
- Two photos instead of three. Myanmar requires three per application. A short set holds up the file.
- A self-completed Report Arrival Form. It is filled out with you at the counter, not in advance.
- An unsigned or undated application. The form must be signed and dated before it is submitted.
- Checked at the counter. Application, Report Arrival Form and photos are reviewed together before anything is filed.
