DS-5504 may have no application fee
If both the passport and legal name change are within one year, eligible applicants use DS-5504 with no application fee. Optional expedited service is $60.
Usually DS-82 if eligible
If more than one year has passed since either the passport was issued or the legal name change, eligible adults generally renew using DS-82 and pay standard renewal fees.
Marriage, divorce or court order
Bring the original or certified copy. It is returned to you with the new passport.

The correct name-change process depends on your eligibility. DS-5504 and eligible DS-82 applications are generally mailed; applicants who do not qualify for those forms may need to apply in person with DS-11.

Who it is for
Passport name change.
Your passport must show the name you currently travel under. Airlines match the booking to the book, not to your marriage certificate.
Which case is yours
- Marriage: bring the certified marriage certificate showing both the previous and new name.
- Divorce: bring the divorce decree that restores or changes the name.
- Court-ordered change: bring the certified court order.
Photocopies and notarized copies are not accepted. Originals and certified copies are returned to you along with the new passport.

Which form, and what it costs
Both the passport and legal name change are within one year. Eligible applicants use Form DS-5504. There is no application fee; expedited service costs $60, and optional 1–3 day return delivery for a passport book is $26.87.
More than one year has passed since either the passport was issued or the legal name change. If you qualify to renew, you generally use DS-82 and pay the standard renewal fee. An expedited adult passport book with 1–3 day return delivery totals $216.87.

How to apply
Seven things to prepare.
Complete the online application
Use DS-5504 when both your passport and legal name change are within one year. If more than one year has passed, use DS-82 if you qualify to renew, or DS-11 if you do not. Follow the signature instructions for the form you are submitting.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Certified proof of your name change
Submit an original or certified copy of the document that legally changed your name. Photocopies and notarized copies are not acceptable.
- Certified marriage certificate.
- Divorce decree showing the name change or restoration.
- Court order granting the name change.

Current U.S. passport
You must submit your current undamaged passport. The old passport will be returned to you along with the new passport.
Identification if DS-11 is required
If you must apply in person using DS-11, bring acceptable identification and the required photocopy. Common examples include:
- Valid driver's license.
- Valid state or government identification.
- Valid military identification.

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa Expedite Services office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Courier authorization and service order forms
If your application qualifies for courier expediting through Sameday Passport & Visa Expedite Services, complete the required authorization and service order forms. The underlying Department of State filing method still depends on whether your case uses DS-5504, DS-82, or DS-11.
