Who it is for
Passport name change.
Your passport must show the name you currently travel under. Airlines match the booking to the book, not to your marriage certificate.
Which case is yours
- Marriage: bring the certified marriage certificate showing both the previous and new name.
- Divorce: bring the divorce decree that restores or changes the name.
- Court-ordered change: bring the certified court order.
Photocopies and notarized copies are not accepted. Originals and certified copies are returned to you along with the new passport.

Which form, and what it costs
Issued less than one year ago. The change is filed on the DS-5504 and the government fee is $60.00.
Issued more than one year ago. The change is filed on the DS-82 as a renewal and the government fee is $213.36.
Government fees are payable to “US Department of State” by cashier's check or money order. Credit cards cannot be accepted for government fees. Sameday Passport & Visa can obtain the money order for you for an additional $10.00 so you can pay everything by debit card, cash or check.

How to apply
Seven things to prepare.
Complete the online application
Fill in the DS-82, or the DS-5504 if your passport was issued less than a year ago, then bring it in unsigned. Do not sign the application until instructed to do so.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Certified proof of your name change
Submit an original or certified copy of the document that legally changed your name. Photocopies and notarized copies are not acceptable.
- Certified marriage certificate.
- Divorce decree showing the name change or restoration.
- Court order granting the name change.

Current U.S. passport
You must submit your current undamaged passport. The old passport will be returned to you along with the new passport.
Proof of identification
Please provide the original of one of the following.
- Valid driver's license.
- Valid state or government identification.
- Valid military identification.

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Applying by mail? Two letters of authorization and a service order form
If you are processing your passport application with Sameday Passport & Visa by mail, please fill out two Letters of Authorization and the service order form, and send them along with the documents.
