Who it is for

A replacement is a new application, so it is filed in person at a Passport Acceptance Facility.
How the in-person appearance works
An appearance in person at a Passport Acceptance Facility is required and a mandatory step of your passport order. After gathering the documents listed below you must take them with you and appear in person, and get a sealed package.
Once you visit the Sameday Passport & Visa office, and have all the documents listed below, you must take them with you and appear in person at a Passport Acceptance Facility, and get a sealed package. When making your appearance, you will pay a Government Fee of $170 and a separate execution fee of $25. Make sure to have a personal check, cashier's check or money order made payable to the “US Department of State” in the amount of $170 with you. The $25 execution fee can be paid by debit card or cash.
They will put all of your documents including your check and original documents in a sealed package and hand it back to you. This sealed package needs to be dropped at the Sameday Passport & Visa office to start the process.
Which case is yours
- Lost or stolen: the DS-64 reports the book so it cannot be used by anyone else.
- Damaged: bring the damaged passport plus a signed statement explaining how it was damaged.
- Passport card: if you lost the book but hold a previously issued card, include it.

How to apply
Seven things to prepare.
Complete the online applications
Fill in the DS-11 online, then bring it in unsigned. Do not sign the application until instructed to do so. An additional form, DS-64, must be completed — it is the statement of your lost or stolen passport.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Proof of citizenship and identification
You must present one of the following identification documents (IDs).
- Certified U.S. birth certificate issued by the city, county, or state, showing the applicant's and parents' details
- Previous expiring or expired U.S. passport, undamaged and unaltered, issued less than 15 years ago
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Citizenship or Naturalization
- Previously issued, undamaged and unaltered U.S. passport issued less than 15 years ago
- Certificate of Naturalization
- Valid driver's license
- Current government ID (city, state, or federal)
- Current military ID (military and dependents)

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Passport card
If you have lost your passport book but have a previously issued passport card, please include it.
Changed your name due to marriage or divorce?
In the event that you changed your name due to marriage or divorce, you will need to bring in a certified copy of your marriage certificate or a court decree that shows your name change.
Applying by mail? Two letters of authorization and a service order form
If you are processing your passport application with Sameday Passport & Visa by mail, please fill out two Letters of Authorization and the service order form, and send them along with the documents.
