Yellow Fever certificate
An international certificate of vaccination against Yellow Fever is required for every Ghana visa. It is checked before the file is submitted.
Capital letters only
The consulate wants two original signed application copies filled out in all capital letters. Both are prepared with you in Dallas.

Single entry is $100.00 and multiple entry is $200.00 at either consulate speed, so the entry type is what moves your total.

Every Ghana visa needs
- Completed application — two original signed copies, filled out in all capital letters
- Valid U.S. passport — current, undamaged, one blank visa page, signed, valid six months from departure
- Copy of valid identification — the address on your driver’s license must match your application, or add a major utility bill
- Yellow Fever certificate — an international certificate of vaccination against Yellow Fever is required
- Two visa photos — 2″ x 2″ on a white background, no older than one month, taken in the Dallas office
How to apply
Tourist visa, everything you bring.
Complete application
- Two original signed copies required.
- Please fill out the form in all capital letters.
Valid U.S. passport
- You must submit your current undamaged passport and it must have one blank visa page.
- The passport must be signed and valid for six months from the date of departure.
Copy of valid identification
- Address on your driver’s license must match the address on your application.
- If the address on your driver’s license does not match your residence, you may include a copy of a major utility bill (water, power, or sewage).
Medical requirements
An international certificate of vaccination against Yellow Fever is required.
Copy of flight itinerary
- If you have a copy of your flight itinerary or ticket, please include it.
- If you do not have a travel itinerary or a ticket, it is okay.
Two visa photos
- Passport photo services are available at the Dallas office.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.
Minors under 18
- Copy of the birth certificate is required.
- Copy of each parent’s passport information page or copy of driver’s licenses.
- Minors not travelling with both parents or guardians must provide a notarized consent signed by the non-accompanying parent or guardian authorizing the consulate to issue a visa.
