How a name change works

To change the name on a passport, you generally need an original or certified document showing the legal name change, such as a marriage certificate, divorce decree, or court order.

If both your passport was issued and your legal name changed less than one year ago, you may be eligible to use Form DS-5504. The DS-5504 application fee is $0; expedited service is optional and costs $60.

Bring the supporting document and current passport to Suite 500 and Sameday Passport & Visa Expedite Services can review which application route applies and help you prepare the submission.

Which route your passport takes

  • Issued less than a year ago: a correction, government fee $60.00.
  • Issued within the last 15 years: a name-change update filed and expedited by Sameday Passport & Visa Expedite Services, government fee $216.87.
  • Issued more than 15 years ago: you will file a new passport application instead.
  • Photocopies and notarized copies of the name-change document are not accepted.

Not sure which one applies? Check both the passport issue date and the date of the legal name change, then call the Dallas office at (972) 231-1600 so the team can review the likely form and fee.
