If one parent cannot attend, have the parent not attending notarize the statement of consent (DS-3053) and provide a copy of their government-issued photo ID.
How a minor's application runs
Sameday Passport & Visa can assist in obtaining a child or minor U.S. passport. An appearance by both parents and the minor at a Passport Acceptance Facility is required and a mandatory step of your passport order.
Once you visit the Dallas office and have all the documents listed below, you take them with you, appear in person at the facility, and get a sealed package. The agent puts all of your documents, including your check and original documents, into that package and hands it back to you.
The sealed package needs to be dropped at Suite 500 to start the process. Do not open it.
Special notes for a minor's passport
- Applicants must be below 16 years of age.
- All children including newborns must have their own passport. A child cannot be added to a parent's book.
- Please remember the child's Social Security number, if they ever had one.
If only one parent can appear, a notarised DS-3053 must be completed along with a copy of the missing parent's ID. Sameday Passport & Visa checks both before you go to the facility.

- Fully valid, undamaged U.S. passport (may be expired).
- A U.S. birth certificate issued by the city, county, or state of birth, listing full name, date and place of birth and the parents' full names, filed with the registrar's office within one year of birth, with the registrar's signature and the seal of the issuing authority.
- Consular Report of Birth Abroad or Certification of Birth.
- Certificate of Naturalization.
- Certificate of Citizenship.

Two authorization letters
The Letter of Authorization will be issued when you come in person to Suite 500. If you are not coming into the Dallas office and are doing it by mail, use the blank form below and leave Courier Company Name blank.
Two letters are needed. The in-person appearance by both parents still applies, so a mailed application starts only once the sealed package reaches the Dallas office.

A child's passport costs less, but carries the execution fee.
The government fee for a minor is $183.36 plus a $35.00 execution fee at the acceptance facility. Return shipping is optional.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. The execution fee can be paid by debit card or cash at the facility. Emergency replacement and limited validity replacement passports depend upon application type.
Sameday Passport & Visa also lets you pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Adding it up for a child
At the fastest speed a minor's passport is the $183.36 government fee, the $35.00 execution fee paid at the acceptance facility, and the $399 service fee, with shipping on top only if you want it couriered.
- Each child needs their own application and their own fees.
- Include a prepaid return air bill and you will not be charged for shipping.
- Provide a FedEx account number and the office will generate the air bill, with a $5 fee added to your work order.
- Or collect the new passport in person at Suite 500.
Government fees + Sameday Passport & Visa expedite fees + return FedEx shipping fees = total fees.
