Turkey is filed as a tourism visa: $60 single entry or $200 multiple entry, valid 3 months.

- Valid passport — signed, 1 blank page, valid 6 months from departure
- Copy of valid ID — with an address matching your application, or a utility bill
- Completed application — signed and dated. Sameday Passport & Visa prepares it with you
- 2x2 visa photo — taken in the Chicago office, under one month old

Employment letter
Original, stating you are allowed vacation time and will be employed on your return to the US. A photocopy is not accepted.
Bank statement
A copy of your most recent statement.
Flight itinerary
If you have one. It is okay if you do not.

Birth certificate, both parents’ passport or license copies, and a notarized consent if not travelling with both parents.

The tourism visa is the only Turkey category filed from the Chicago office. The choice you make is how many entries you need inside the 3 month window.

What gets checked before submission
Four things, and all four are common refusal reasons.
- Employment letter — must be an original, not a copy, and must state approved vacation time and continued employment
- Bank statement — must be your most recent statement
- ID address — must match the address on your application, or add a utility bill
- Photo age — 2x2 inches, under one month old. Photos are taken at the Chicago counter
