Every Ivory Coast visa needs
- Completed application — printed, signed and dated
- Valid U.S. passport — current, undamaged, one blank visa page, signed, valid six months from departure
- Two visa photos — 2″ x 2″ on a white background, no older than one month, taken in the Chicago office
- Yellow Fever vaccination — a copy or the original certificate is required
- Itinerary and hotel — include a copy of each if you have them, and it is okay if you do not

Complete application

Valid U.S. passport
- You must submit your current undamaged passport and it must have one blank visa page.
- The passport must be signed and valid for six months from the date of departure.
Two visa photos
- Passport photo services are available at the Chicago office.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.
Copy of flight itinerary
- If you have a copy of your flight itinerary or ticket, please include it.
- If you do not have a travel itinerary or a ticket, it is okay.
A copy of Yellow Fever vaccination
You are required to submit a copy or the original certificate of Yellow Fever vaccination.
Proof of hotel reservations
- If you have a copy of your hotel reservations, please include them.
- If you do not have reservations prepared, it is okay.
