Every Indonesia visa needs
- Completed application — filled in and checked with the Chicago office
- Valid U.S. passport — current, undamaged, one blank visa page, signed, valid six months from departure
- Copy of valid identification — the address on your driver’s license must match your application, or add a major utility bill
- Copy of flight itinerary — include it if you have one, and it is okay if you do not
- Visa photos — 2″ x 2″ on a white background, no older than one month, taken in the Chicago office
How to apply
Tourist visa, everything you bring.
Complete application
Fill in the Indonesia visa application and bring it to the Chicago office, or send it with your documents.
Valid U.S. passport
- You must submit your current undamaged passport and it must have one blank visa page.
- The passport must be signed and valid for six months from the date of departure.
Copy of valid identification
- Address on your driver’s license must match the address on your application.
- If the address on your driver’s license does not match your residence, you may include a copy of a major utility bill (water, power, or sewage).
Copy of flight itinerary
- If you have a copy of your flight itinerary or ticket, please include it.
- If you do not have a travel itinerary or a ticket, it is okay.
Original and copy of birth certificate
The birth certificate must be notarized, and then legalized by the consulate. Start this step early, because it adds time before the visa itself is filed.
Visa photos
- Passport photo services are available at the Chicago office.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.
Minors under 18
- Copy of the birth certificate is required.
- Copy of each parent’s passport information page or copy of driver’s licenses.
- Minors not travelling with both parents or guardians must provide a notarized consent signed by the non-accompanying parent or guardian authorizing the consulate to issue a visa.

- Business letter — on the company letterhead with an original signature, stating your name, position, nature of work, how many days, where you are staying and who will provide the financial guarantee, with the name, address and phone number of the contact person
- Bank statement — a copy of your most recent bank statement
- Valid U.S. passport — undamaged, one blank visa page, signed, six months validity from departure
- Copy of valid identification — matching address, or a major utility bill
- Visa photos — 2″ x 2″, white background, no older than one month
Work visa
Everything on the business file, plus two more documents.
- Invitation letter — original or scanned copy, on the company’s letterhead with the company seal or the red seal from the official foreign ministry
- Copy of hotel reservation — required with the work visa file
- Business letter — same wording requirements as the business visa
- Copy of flight itinerary — if you have one. It is okay if you do not
- Visa photos — 2″ x 2″, white background, no older than one month
