Eligibility
You can renew by mail if all of the following are true.
- Your most recent passport is a 10-year adult passport (issued age 16 or older)
- It expired no more than 5 years ago, or will expire within 5 years
- It is undamaged and can be submitted with your application
- You were over 16 when it was issued and your appearance has not significantly changed
If any line above does not describe your passport, it is filed as a new application instead. Sameday Passport & Visa will confirm which track you are on over the phone.

What to send
Everything that goes in the envelope.
- Your original, undamaged passport — a 10-year passport, expired no more than 5 years ago
- A copy of your driver’s license or state ID — front and back, if possible
- Two passport photos — 2x2 inches, on a white background
- A personal check or money order for $213.36 — payable to the U.S. Department of State
- A travel itinerary — if you are traveling within 14 days
- Form DS-82 — completed, printed, and signed
- Two Letters of Authorization — of the same kind (check the first two options, leave the courier company blank)
- Service Order Form — completed and printed
- Do not mail anything until you have spoken with Sameday Passport & Visa first

Form DS-82
Fill out Form DS-82, print it, and sign it. Unlike the DS-11 for new passports, you sign the DS-82 yourself before mailing — no acceptance facility required.
Service Order Form
Print and complete the Service Order Form and include it with your documents.
Two Letters of Authorization
Fill out two letters of the same kind. Check the first two boxes only, and leave the courier company blank until you call Sameday Passport & Visa.
Personal check or money order
Make it payable to the U.S. Department of State. The adult renewal government fee is $213.36. Do not send cash.
Travel itinerary
If you have a ticket or itinerary showing travel within 14 days, include a copy. If you do not have one yet, call Sameday Passport & Visa so the timing can be advised.
Passport photos
Two photos, 2x2 inches, on a white background, taken within the last month. Sameday Passport & Visa takes compliant photos in the Chicago office if you want to stop by first.
Changed your name?
If you changed your name through marriage or divorce, include a certified marriage certificate or court decree. Originals or certified copies only — no photocopies or notarized copies.
Proof of identity
Include a clear copy, front and back, of one valid ID:
- A valid U.S. driver’s license or state ID
- A U.S. military ID
- A permanent resident card (if you are not a U.S. citizen)

Government expedite fees must be paid by cashier’s check or money order, payable to “US Department of State”. Emergency replacement and limited validity replacement passports depend upon application type.
Sameday Passport & Visa also lets clients pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Mailing in, and what it costs you
- Return shipping is optional. Sameday Passport & Visa only returns passports using FedEx.
- Include a prepaid return air bill and there is no FedEx charge from Sameday Passport & Visa.
- Give a FedEx account number and Sameday Passport & Visa generates the air bill for an additional $5 on your work order.
- Passport photos can be taken at the Chicago office if you stop in before mailing.
