Replace a lost, stolen, or damaged passport
Lost and stolen passports should be reported as soon as possible and replaced in person with Form DS-11. A damaged passport follows a different replacement path: bring the damaged passport itself and a signed statement explaining the damage. We can review the correct documents before you apply.

The in-person step, explained
An appearance in person at a Passport Acceptance Facility is required and is a mandatory step of your passport order.
Once you visit the Arlington office and have all the documents listed below, you take them with you, appear in person at a Passport Acceptance Facility, and get a sealed package. At that appearance you pay the government fee and a separate execution fee.
They put all of your documents, including your check and original documents, in a sealed package and hand it back to you. That sealed package needs to be dropped at the Arlington office to start the process.
Damaged rather than lost?
A damaged passport is not reported as lost or stolen. To replace it, apply in person using Form DS-11, bring the damaged passport, and include a signed statement explaining the damaged or mutilated condition.
Statement of damaged passport (PDF)
Bring the damaged passport itself. Do not discard it, because the Department of State requires it with the replacement application.
No proof of citizenship at all? Request a file search and Sameday Passport & Visa Expedite Services will guide you from there.

Applying by mail?
If you live outside Arlington and plan to send a sealed DS-11 package to us after your acceptance-facility appointment, contact us first so we can confirm the authorization and service-order documents required for the private courier step.
The in-person DS-11 acceptance step still applies. Mailing documents to a private passport service does not replace the required appearance at an authorized passport acceptance facility.
Fees & processing times
A replacement carries the execution fee.
Because the application is filed in person, the $35.00 execution fee applies on top of the government fee. Return shipping is optional.
