If one parent cannot attend, have the parent not attending notarize the statement of consent (DS-3053), and include a copy of the missing parent's ID.

How a minor's application works
Sameday Passport & Visa can assist in obtaining a child or minor U.S. passport. An appearance by both parents and the minor at a Passport Acceptance Facility is required and is a mandatory step of your passport order. If only one parent can appear, a Special Circumstances DS-3053 form must be completed and notarized along with a copy of the missing parent's ID.
Once you visit the Arlington office and have all the documents listed below, you take them with you and appear in person at a Passport Acceptance Facility to get a sealed package. They will put all of your documents, including your check and original documents, in a sealed package and hand it back to you. This sealed package needs to be dropped at our office to start the process.
Special notes for a minor
- Applicants must be below 16 years of age.
- All children, including newborns, must have their own passport.
- Please remember the child's Social Security number, if they have ever had one.
- At 16 and older, the first time or renewal process applies instead.
Bring a personal check, cashier's check or money order made payable to “US Department of State” for the government fee. The execution fee can be paid by debit card or cash.

The Letter of Authorization will be issued when you come in person. If you are not coming into the Arlington office and are doing it by mail, use the form below and leave Courier Company Name blank.

A child's passport costs less than an adult's.
The government fee for an applicant under 16 is lower than the adult fee, but the $35.00 execution fee still applies because minors are filed in person.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. Emergency replacement and limited validity replacement passports depend upon application type.
Sameday Passport & Visa also lets you pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Travelling as a family?
Each child needs their own passport and their own government fee, newborns included. Filing everyone in the same visit means one trip to the acceptance facility and one set of photos taken at Suite 108.
- Include a prepaid return air bill and you will not be charged for shipping.
- Provide a FedEx account number and the office will generate the air bill, with a $5 fee added to your work order.
- Or collect the passports in person on N Cooper Street.
Government fees + Sameday Passport & Visa expedite fees + return FedEx shipping fees = total fees.
