Original tourist support and voucher
From a licensed Belarus travel agency. The confirmation must be original.
Travel insurance letter
With the insurer's name, address and phone, your name, policy number, coverage and validity dates.
Former USSR or Belarus citizens
Submit documents proving the loss of citizenship.
Minors under 18
Birth certificate, both parents' passport or license copies, and a notarized consent if not traveling with both parents.
By mail?
Include a Service Order Form with your documents and Sameday Passport & Visa files at the consulate for you.

The consulate reads the insurer's letter, not the card. Sameday Passport & Visa checks all six items before your file is filed.
