The invitation is checked
The tourist confirmation and voucher from the licensed Belarus travel agency must be an ORIGINAL. Sameday Passport & Visa verifies it before your file goes in.

Every Belarus visa needs

- Valid U.S. passport — current, undamaged, one blank visa page, signed, valid six months from departure
- Two visa photos — 2″ x 2″ on a white background, no older than one month, taken in the Chicago office
- Travel insurance — valid in Belarus or worldwide, minimum $13,000 coverage with $0 deductible
- Former USSR or Belarus citizens — documents proving the loss of USSR or Belarusian citizenship

Complete application
After completing the application form, please print it. Please do not forget to sign and date your application.
Valid U.S. passport
- You must submit your current undamaged passport and it must have one blank visa page.
- The passport must be signed and valid for six months from the date of departure.
Two visa photos
- Passport photo services are available at the Chicago office.
- Must be 2″ x 2″ on a white background.
- Photo must not be older than one month.
ORIGINAL tourist visa support or invitation
Standard Tourist Confirmation and voucher from the licensed Belarus travel agency. The confirmation must be ORIGINAL.
Minors under 18
- Copy of the birth certificate is required.
- Copy of each parent’s passport information page or copy of driver’s licenses.
- Minors not travelling with both parents or guardians must provide a notarized consent signed by the non-accompanying parent or guardian authorizing the consulate to issue a visa.
Former USSR or Belarus citizens
If you formerly had USSR or Belarusian citizenship, please submit the documents proving the loss of USSR or Belarusian citizenship.
Travel insurance
All visa applicants have to present medical insurance valid for Belarus or worldwide. A letter from your insurance company, original or copy, is required, showing the name, address and phone number of the insurance company; the full name of the policy holder; the policy number; territorial coverage valid in Belarus or worldwide; a term of validity covering the whole period of travel or temporary residence in Belarus; and medical coverage of not less than 13,000 US Dollars with $0 deductible. A photocopy of an insurance card is not sufficient.

Letter of invitation to Belarus
Original or a copy, but every element below is required.
- Registered inviting party — an agency, company or organization in the Republic of Belarus registered as a legal entity by the Belarusian authorities
- On official letterhead — with a registration number, date of issue, the signature of an official and a corporate seal
- Duration of stay — the letter must indicate the expected duration of your stay in Belarus
- Written guarantees — the inviting party must guarantee that you will abide by the rules and regulations applied to foreign citizens in the Republic of Belarus
