Which route applies to you
In order to change the name on your passport you must have an original court ordered name change, original marriage certificate or divorce decree.
You must renew your passport in order to change your name. If your passport was issued less than a year from the date of issue and you need your name changed or corrected, you will only need to pay the expedite fee to the U.S. State Department to have it corrected.
Issued more than a year ago and within the last 15 years, the change goes through as a renewal at the full government fee. Issued over 15 years ago, you file a new passport instead.
What the certified document has to be
An original or certified copy from the issuing authority, not a photocopy and not a scan. A commemorative marriage certificate from the ceremony will not be accepted.
For a divorce, the decree page showing the restored name is the one that matters. For an adoption or a court-ordered change, the signed order.
Sameday Passport & Visa returns your originals along with the new passport, so nothing you bring to Suite 108 is kept.

Complete the online application
DS-5504 is the form when your passport was issued less than a year ago. Beyond a year the change is filed on the DS-82 renewal form. Sameday Passport & Visa confirms which one applies from your issue date.

If you are processing your passport application with Sameday Passport & Visa by mail, fill out two Letters of Authorization and send them along with the documents, plus the service order form.
Send the certified name-change document with the application. Sameday Passport & Visa returns it to you with the new passport.

Under a year since issue, $60.00. Over a year, $213.36, the same as a renewal. The Sameday Passport & Visa service fee and optional FedEx shipping are separate.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. Emergency replacement and limited validity replacement passports depend upon application type.
Sameday Passport & Visa also lets you pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Adding it up for a name change
A recent passport at the fastest speed is $60.00 to the government plus the $399 service fee. Beyond a year it is $213.36 plus the service fee, with shipping on top only if you want it couriered.
- Include a prepaid return air bill and you will not be charged for shipping.
- Provide a FedEx account number and the office will generate the air bill, with a $5 fee added to your work order.
- Or collect the new passport in person at Suite 108.
Government fees + Sameday Passport & Visa expedite fees + return FedEx shipping fees = total fees.
