A lost, stolen, or damaged passport must be reported and replaced in person. Sameday Passport & Visa files the DS-64 with your DS-11 application. If your passport is damaged, bring a signed statement explaining how.

The in-person step, explained
An appearance in person at a Passport Acceptance Facility is required and is a mandatory step of your passport order.
Once you visit the Arlington office and have all the documents listed below, you take them with you, appear in person at a Passport Acceptance Facility, and get a sealed package. At that appearance you pay the government fee and a separate execution fee.
They put all of your documents, including your check and original documents, in a sealed package and hand it back to you. That sealed package needs to be dropped at the Arlington office to start the process.
Damaged rather than lost?
A mutilated or water-damaged book is replaced on the same route as a lost one, with one addition: a signed statement describing how the damage happened.

Bring the damaged passport itself. Sameday Passport & Visa submits it with the application, so do not discard it.

If you are processing your passport application with Sameday Passport & Visa by mail, fill out two Letters of Authorization and send them along with the documents, plus the service order form.
Remember that the in-person appearance still applies, so a mailed replacement starts only once the sealed package reaches the Arlington office.

A replacement carries the execution fee.
Because the application is filed in person, the $35.00 execution fee applies on top of the government fee. Return shipping is optional.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. Emergency replacement and limited validity replacement passports depend upon application type.
Sameday Passport & Visa also lets you pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Adding it up for a replacement
At the fastest speed a replacement is the $213.36 government fee, the $35.00 execution fee paid at the acceptance facility, and the $399 service fee, with shipping on top only if you want it couriered.
- Include a prepaid return air bill and you will not be charged for shipping.
- Provide a FedEx account number and the office will generate the air bill, with a $5 fee added to your work order.
- Or collect the new passport in person at Suite 108.
Government fees + Sameday Passport & Visa expedite fees + return FedEx shipping fees = total fees.
