How a replacement runs in Dallas
An appearance in person at a Passport Acceptance Facility is required and a mandatory step of your passport order. After gathering the documents listed below you take them with you, appear in person at the facility, and get a sealed package.
For an expedited adult passport book, the Department of State amount is $213.36 when the $130 application fee, $60 expedite fee and $23.36 optional 1–3 day passport-book delivery all apply. A separate $35 acceptance fee is paid to the acceptance facility. The Department of State fee is generally paid by check or money order payable to “U.S. Department of State”; payment methods for the $35 acceptance fee depend on the facility.
The agent puts all of your documents, including your check and original documents, into a sealed package and hands it back to you. That package needs to be dropped at Suite 500 to start the process. Do not open it.
Reporting the passport as lost or stolen
- The DS-64 officially reports the old book and invalidates it, so it cannot be misused.
- It is filed together with the DS-11 replacement application, never on its own.
- Lost the book but hold a previously issued passport card? Include the card.
- Damaged book: bring it in with a signed statement explaining how it was damaged.

If you are processing your passport application with Sameday Passport & Visa Expedite Services by mail, fill out two Letters of Authorization and the service order form and send them along with the documents to Suite 500.

The Department of State application fee is generally paid by check or money order payable to “U.S. Department of State”. The separate $35 DS-11 acceptance fee is paid to the acceptance facility, using the payment methods that facility accepts.
Sameday Passport & Visa Expedite Services also lets you pay everything by debit card, cash or check and will obtain the money order for an additional fee of $10.00.

Adding it up for a replacement
At the fastest listed private service level, an expedited adult passport book is $213.36 in Department of State fees when optional 1–3 day passport-book delivery is included, plus the separate $35 acceptance fee and the $399 private service fee. Private return shipping is additional only if selected.
- Include a prepaid return air bill and you will not be charged for shipping.
- Provide a FedEx account number and the Dallas office will generate the air bill, with a $5 fee added to your work order.
- Or collect the new passport in person at Suite 500.
- Return shipping is an optional service. Sameday Passport & Visa Expedite Services only returns passports using FedEx.
