If you cannot come into our Los Angeles office and want to apply for a new or first-time passport by mail, please follow these steps. New passport applications require an in-person appearance at an acceptance facility before mailing your documents to us.

Step by step
How the mail-in process works
Three things happen before your package reaches Sameday Passport & Visa Expedite Services. Get them right and nothing is delayed.

Take all of your documents to an acceptance facility before mailing them to our office. An acceptance facility is an office authorized by the U.S. Department of State to execute your passport application.
- 2
Sign your application in front of them. They will charge you a $35 execution fee.
- 3
They will put all of your documents in an envelope, seal it, and return it to you.
Do not open the envelope after
There is only a selected office you can go to, but Sameday Passport & Visa Expedite Services will assist you in finding one.
Your package
What to send
- Proof of U.S. citizenship: original long-form birth certificate, Naturalization certificate, or a previous U.S. passport
- A copy of your driver's license or ID, front and back (a child needs both parents' IDs)
- Two passport photos, 2 x 2 inches, on a white background
- A personal check or money order: $216.87 adult, or $186.87 child, payable to the U.S. Department of State
- A travel itinerary if you are traveling within 14 days
- Form DS-11, completed and printed (do not sign it yet)
- Two Letters of Authorization of the same kind
- A completed Service Order Form

Details that matter
Good to know
Most mail-in packages are delayed by a form signed too early or a birth certificate that is not the long form. Check these before you seal anything.
Complete the DS-11
Fill out Form DS-11, then print it. Do not sign it until an acceptance agent instructs you to. You must provide a Social Security number if you have one.
Two Letters of Authorization
Check the first two boxes only and do not fill out the courier company until you call Sameday Passport & Visa Expedite Services. Include one letter in your package, and send the second with the package after the acceptance facility seals it. For a minor (16 or under), both parents sign.
Service Order Form
Print and complete the Service Order Form and send it with the extra Letter of Authorization and the sealed package.
Proof of U.S. citizenship
Provide an original or certified copy of one of the following:
- A certified birth certificate, long form (not abstract)
- A Consular Report of Birth Abroad
- A Certificate of Naturalization
- For a minor, a certified long-form birth certificate is required, plus a previous passport if available
Personal check or money order
Payable to the U.S. Department of State: $216.87 for an adult, or $186.87 for a child under 16. Do not send cash.
Copy of valid ID
A front and back copy of your driver's license or state ID. For a child, include copies of both parents' IDs.
Passport photos
Two passport photos, 2x2 inches, white background, taken within the last 6 months. Sameday Passport & Visa Expedite Services can take them if you visit our office first, $20.
Include a copy of your itinerary in the package and call Sameday Passport & Visa Expedite Services first, so the filing speed is matched to your departure date.

Government payment methods depend on how and where you apply. For a first-time DS-11 passport by mail, the U.S. Department of State payment for an expedited passport book with 1–3 day return delivery is $216.87 for an adult or $186.87 for a child under 16. The separate $35.00 acceptance fee is paid to the acceptance facility. Sameday Passport & Visa Expedite Services can help confirm the exact payment instructions before you mail your sealed package.

Optional Service. Sameday Passport & Visa Expedite Services only returns passports using FedEx. You can include a prepaid return air bill in which case you will not be charged. You can provide a FedEx account number and Sameday Passport & Visa Expedite Services will generate the air bill — an additional fee of $5 will be charged to your work order.
