Limited-Validity Passport Help in Los Angeles

- Your current limited validity U.S. passport, undamaged. It is submitted with the application
- Correct form: DS-5504, DS-11, or DS-82 depending on why the passport was limited and when it was issued
- Proof of citizenship if the original application was short of documents: original birth certificate or naturalization certificate

A limited-validity passport can often be replaced with a full-validity passport after you resolve the issue that caused the limitation and follow the form instructions for your case.
Why a passport gets limited validity
- Emergency issuance: a book issued quickly for urgent travel.

- Incomplete documents: citizenship evidence was short at the time of application.

How the exchange works
Sameday Passport & Visa Expedite Services reviews the passport endorsement, why it was limited, when it was issued, and your supporting documents to determine whether your case uses DS-5504, DS-11, or DS-82.
If the limit was set because citizenship evidence was incomplete, that evidence must be supplied now — an original birth certificate with a seal, a Consular Report of Birth Abroad, or a Certificate of Naturalization or Citizenship.
Government fees depend on the form. Some eligible DS-5504 replacements require no application fee unless you add optional expedited service. DS-11 and DS-82 cases use the applicable standard passport fees. We confirm the fee path before submission.
How to apply
Seven things to prepare.
Complete the online application
Use the form required for your specific limited-validity situation. Some cases use DS-5504, while others require DS-11 or may qualify for DS-82. We review the passport and supporting documents before you submit the application.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Your limited validity passport
You must submit your current undamaged passport. The limited book will be returned to you along with the new passport.
Proof of citizenship
If the limit was set because citizenship evidence was incomplete, that evidence must be supplied now. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.

Proof of identification
Please provide the original of one of the following.
- Valid driver's license.
- Valid state or government identification.
- Valid military identification.

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa Expedite Services office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Courier authorization and service order forms
If your application qualifies for courier expediting through Sameday Passport & Visa Expedite Services, complete the required authorization and service order forms. The government filing method still depends on whether your case uses DS-5504, DS-11, or DS-82.
