Lost or stolen passports
Report a valid lost or stolen passport immediately. You can report it online, by mail, or in person when you apply for a replacement.

A replacement is a new application, so it is filed in person at a Passport Acceptance Facility.
How the in-person appearance works
A replacement for a lost, stolen, or significantly damaged passport generally requires Form DS-11 and an in-person appearance at an authorized Passport Acceptance Facility or passport agency. The acceptance agent verifies your identity, witnesses your signature, and accepts the application.
After we review the documents listed below, take your application to an authorized Passport Acceptance Facility. For an expedited adult passport book replacement, current Department of State charges total $216.87 when the $130 application fee, $60 expedite fee, and $26.87 1–3 day return delivery fee apply. The separate execution/acceptance fee is $35 and is paid to the acceptance facility.
If the acceptance facility provides a sealed package for eligible courier expediting, do not open it. Bring the sealed package back to Sameday Passport & Visa Expedite Services so the expedited handling process can continue.
Which case is yours
- Lost or stolen: report a valid passport immediately so the U.S. Department of State can cancel it. A replacement requires a new in-person application.
- Damaged: bring the damaged passport plus a signed and dated statement explaining the damage. Significant damage generally requires a new DS-11 application.
- Passport card: if you lost the book but hold a previously issued card, include it.

How to apply
Seven things to prepare.
Complete the online applications
Complete Form DS-11 and bring it unsigned. Do not sign it until the passport acceptance agent instructs you to do so. If your valid passport was lost or stolen, report it immediately; you can report it online, by mail using Form DS-64, or in person when applying for the replacement.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Proof of citizenship and identification
You must present one of the following identification documents (IDs).
- Certified U.S. birth certificate issued by the city, county, or state, showing the applicant's and parents' details
- Previous expiring or expired U.S. passport, undamaged and unaltered, issued less than 15 years ago
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Citizenship or Naturalization
- Previously issued, undamaged and unaltered U.S. passport issued less than 15 years ago
- Certificate of Naturalization
- Valid driver's license
- Current government ID (city, state, or federal)
- Current military ID (military and dependents)

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa Expedite Services office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Passport card
If you have lost your passport book but have a previously issued passport card, please include it.
Changed your name due to marriage or divorce?
In the event that you changed your name due to marriage or divorce, you will need to bring in a certified copy of your marriage certificate or a court decree that shows your name change.
Courier authorization and service order forms
If your application qualifies for courier expediting through Sameday Passport & Visa Expedite Services, complete the required Letter of Authorization and service order form. The DS-11 application itself still requires the appropriate in-person acceptance step.

For DS-11 applications, the U.S. Department of State application payment and the $35 execution/acceptance fee are separate payments. State Department fees are generally paid by check or money order payable to “U.S. Department of State”.
