Every child needs their own
All children including newborns must have their own passport — a child cannot travel on a parent's book.

Both parents usually approve
If one parent cannot attend, a notarized DS-3053 and copy of that parent's ID may be required. Sole-custody and other exceptions use different documents.

Sameday Passport & Visa Expedite Services can assist in obtaining a child/minor US passport for any applicant under 16.
How the in-person appearance works
A child under 16 must apply in person at an authorized Passport Acceptance Facility. Both parents or guardians generally must approve issuance. If one parent cannot attend and both share custody, the absent parent generally completes and notarizes Form DS-3053 and provides a photocopy of the ID used with the notary. Sole-custody and other situations require different supporting documents.
After we review the documents listed below, take the application to an authorized Passport Acceptance Facility. For an expedited minor passport book, the current Department of State charges total $186.87 when the $100 application fee, $60 expedite fee, and $26.87 1–3 day return delivery fee apply. The separate execution/acceptance fee is $35 and is paid to the acceptance facility.
If the acceptance facility provides a sealed package for eligible courier expediting, do not open it. Bring the sealed package back to Sameday Passport & Visa Expedite Services so the expedited handling process can continue.
Special notes regarding a passport for a minor
- Applicants must be below 16 years of age.
- All children including newborns must have their own passport.
Please remember the child's Social Security Number (SSN), if they ever had one.

How to apply
Seven things to prepare.
Complete the online application
Fill in the DS-11 online, then bring it in unsigned. Do not sign the application until instructed to do so.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Proof of citizenship
You must submit one of the following. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.
- Fully-valid, undamaged U.S. passport (may be expired)
- U.S. birth certificate issued by the city, county, or state of birth — listing the child's full name, date of birth and place of birth, the parent(s)' full names, the date filed with the registrar's office (within one year of birth), the registrar's signature, and the seal of the issuing authority
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Proof of identification
Parent(s) must present one of the following identification documents (IDs).
- Fully-valid U.S. driver's license
- Fully-valid, undamaged U.S. passport (may be expired)
- Certificate of Naturalization or Citizenship
- Government employee ID (city, county, state or federal)
- U.S. military ID or military dependent ID
- Valid foreign passport
- Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

Current US passport
If a passport has been previously issued, please bring it along. You must submit the current undamaged passport. The expired passport will be returned to you along with the new passport.
Two authorization letters
A Letter of Authorization will be issued when you come in person. If you are not coming into the Sameday Passport & Visa Expedite Services office and are doing it by mail, use the blank letter below and leave Courier Company Name blank.
Provide a flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, please inform Sameday Passport & Visa Expedite Services.
Passport photos
Passport photo services are available at the Sameday Passport & Visa Expedite Services office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.

For DS-11 applications, the U.S. Department of State application payment and the $35 execution/acceptance fee are separate payments. State Department fees are generally paid by check or money order payable to “U.S. Department of State”.
