- Proof of citizenship: original birth certificate, naturalization certificate, or a previous U.S. passport
- Remember the Social Security number of the applicant
- Valid ID: a valid state ID, Military ID, or foreign passport
- Government fee: $213.36 by Personal check (with printed address), cash, or debit card. Credit cards are not accepted. Make check payable to US DEPARTMENT OF STATE.
- Name change documents (if applicable): original or certified marriage certificate, divorce decree, or court order. No photocopies or notarized copies

Who it is for
New or first time passport.
To obtain a new or first time passport you must meet one of the following requirements.
You qualify if
- You are applying for your first U.S. passport
- Your previous U.S. passport was issued when you were under age 16
- Your previous U.S. passport was issued more than 15 years ago
If your passport expired within the last 15 years and you were 16 or older when it was issued, you qualify for passport renewal instead, which has a simpler process.
Applying for a child under 16? See child or minor passport. Replacing a passport that is gone or damaged? See lost, stolen or damaged.
The sealed package
Once you visit the Sameday Passport & Visa office and have all the supporting documents, you must appear in person at a Passport Acceptance Facility and get a sealed package. This sealed package needs to be dropped at the office to start the process.
When making your appearance, you will pay a Government Fee and a separate execution fee. They will put all of your documents including your check and original documents in a sealed package, and hand it back to you.
Government Fees can be paid via checks (personal, certified, cashier's, traveler's) and money orders payable to “U.S. Department of State”. Credit and debit cards CANNOT be accepted.
Execution Fees can be paid via personal checks, money orders and credit or debit cards.
How to apply
Eight things to prepare.
Complete the online application
Fill in the DS-11 online, then bring it in unsigned. Do not sign the application until instructed to do so.
You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
Proof of U.S. citizenship
Submit one of the following. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.
- Fully-valid, undamaged U.S. passport (may be expired)
- U.S. birth certificate issued by the city, county or state of birth, listing your full name, date and place of birth and your parents' full names, filed with the registrar within one year of birth, with the registrar's signature and the seal of the issuing authority
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Present ID
You must present one of the following identification documents.
- Fully-valid U.S. driver's license
- Government employee ID (city, county, state or federal)
- U.S. military ID or military dependent ID
- Valid foreign passport
- Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

Flight itinerary
If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay.
Passport photos
Passport photo services are available at the Sameday Passport & Visa office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.
Changed your name due to marriage or divorce?
In the event that you changed your name due to marriage or divorce, you will need to bring in a certified copy of your marriage certificate or a court decree that shows your name change.
