Renewal and DS-11 applications follow different steps.
Before paying fees or preparing documents, confirm which form and in-person requirements apply to your passport situation.
Eligible adult renewal
Many adults who qualify to renew use Form DS-82. A qualifying renewal does not use the same acceptance-facility process as a first-time DS-11 application.
- Bring or provide your most recent passport information.
- Confirm that you meet the current DS-82 renewal requirements.
- Prepare a compliant passport photo and any required name-change evidence.
- Give the Chicago team your actual departure date before choosing a private service speed.
First-time, child or other DS-11 case
First-time adult applicants, children under 16, and many applicants who cannot use DS-82 file with Form DS-11 and must appear in person at an authorized acceptance facility.
- Citizenship evidence and acceptable identification are required.
- The application is signed when instructed during the in-person process.
- A separate acceptance-facility fee applies to DS-11 applications.
- The Chicago office can review the paperwork before submission to help identify missing items.
How the Chicago process works
Start with the passport type and your travel date.
The Chicago team reviews the filing route first so your form, supporting documents, and service choice match your actual situation.
- STEP 1
Explain your passport need
Tell us whether this is a renewal, first passport, minor application, replacement, name change, or another passport request.
- STEP 2
Review the paperwork
We check the application type, photo, citizenship or identity evidence, and supporting records before the required filing step.
- STEP 3
Complete the correct filing step
DS-11 applicants complete the required in-person acceptance process. Eligible renewals use the applicable renewal route.
- STEP 4
Continue with the selected service
Once the required documents are ready, the Chicago team confirms the private service option and next handling step for your case.
