How a replacement runs in Chicago

An appearance in person at a Passport Acceptance Facility is required and a mandatory step of your passport order. After gathering the documents listed below you take them with you, appear in person at the facility, and get a sealed package.

Once you visit the Chicago office and have all the documents ready, you will pay a government fee of $213.36 and a separate execution fee of $35.00 at the facility. Have a personal check, cashier's check or money order made payable to “US Department of State” in the amount of $213.36 with you. The execution fee can be paid by debit card or cash.

The agent puts all of your documents, including your check and original documents, into a sealed package and hands it back to you. That package needs to be dropped at Suite 203 to start the process. Do not open it.

Reporting the passport as lost or stolen

  • The DS-64 officially reports the old book and invalidates it, so it cannot be misused.
  • It is filed together with the DS-11 replacement application, never on its own.
  • Lost the book but hold a previously issued passport card? Include the card.
  • Damaged book: bring it in with a signed statement explaining how it was damaged.

Sameday Passport & Visa checks both forms before you leave for the acceptance facility, so a missing signature or a blank field does not send you back to Suite 203 a second time.

What to have ready before you come in.

Complete the online applications
Do not sign the application until instructed to do so. You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f). An additional form, DS-64, must be completed.

Proof of citizenship and identification
You must present one of the following identification documents:
  • Fully-valid U.S. driver's license.
  • Fully-valid, undamaged U.S. passport (may be expired).
  • Certificate of Naturalization or Citizenship.
  • Government employee ID (city, county, state or federal).
  • U.S. military ID or military dependent ID.
  • Valid foreign passport.
  • Matricula Consular, commonly used by the parent of a U.S. citizen child applicant.

Changed your name due to marriage or divorce?
In the event that you changed your name due to marriage or divorce, you will need to bring in a certified copy of your marriage certificate or a court decree that shows your name change.

If you have a copy of your flight itinerary or ticket, please include it. If you do not have a travel itinerary or a ticket, it's okay — tell Sameday Passport & Visa when you come in.
Passport photos
Passport photo services are available at the Chicago office. Photos must be 2″ x 2″ on a white background, and must not be older than one month.

Government expedite fees must be paid by cashier's check or money order, payable to “US Department of State”. The execution fee can be paid by debit card or cash at the facility. Emergency replacement and limited validity replacement passports depend upon application type.

At the fastest speed a replacement is the $213.36 government fee, the $35.00 execution fee paid at the acceptance facility, and the $399 service fee, with shipping on top only if you want it couriered.

  • Include a prepaid return air bill and you will not be charged for shipping.
  • Provide a FedEx account number and the Chicago office will generate the air bill, with a $5 fee added to your work order.
  • Or collect the new passport in person at Suite 203.
  • Return shipping is an optional service. Sameday Passport & Visa only returns passports using FedEx.
