· Valid and signed U. S. Passport

· One (1) Visa Application Form.

· Two (2) Passport type photograph

· A Medical report or doctor's letter is required, confirming absence of any contagious diseases

· Letter of Invitation is required ( fax copy ) , if Visiting friends /relatives.

· Copy of travel itinerary.
