·         Online Visa Application sign and dated.

·         Two (2) Passport type photograph

·         Copy of the birth certificate if born in US or a copy of nationalization certificate

·         Applicants must submit one clear copy of driver’s license or their utility bill, For Minor,
copy of parent's driver license is acceptable

·         Non US Citizen, You must send us a copy of both sides of your U.S. Green Card or your U.S. visa.

·         Resume: Submit a copy of applicant's resume.
