· One (1) Visa Application Form.

· Three(2) Passport type photograph

· World Health Certificate required showing valid vaccinations against Yellow Fever

· Copy of Bank Statement or Bank Letter with details that applicant has sufficient funds his travel needs

· Notarized and legalized letter of invitation from the applicant's sponsor is required if the duration of stay is over 30 days

· Copy of Travel itinerary.

· Business Letter from U.S. Company address to "Consulate of Burundi, explaining the purpose of the trip, guaranteeing financial support for you while in Burundi.

· Notarized and legalized letter of invitation from the applicant's sponsor is required if the duration of stay is over 90 days.

· Copy of Travel itinerary.
