Lost or Stolen U.S. Passport
REQUIREMENTS TO EXPEDITE A LOST 0R STOLEN US PASSPORT
INSTRUCTIONS:
- Complete DS-11 passport application form, fill it online. But do not sign.
- Original/certified US birth certificate or Original naturalization certificate.
- Proof of Identity (A Valid Driving License, Government Issue Photo I.D).
- If your passport was lost or stolen: complete form DS-64, must be signed and dated.
- SameDay Passport authorization letter, Please call
